TT2112 form: what it is for and how to fill it in

The TT2112 starts several driving-licence procedures: choose the right version and check the fields and attachments before submitting it.

TT2112 is an application form used for driving-licence applications and other procedures. The correct version, fields and attachments depend on the procedure you are applying for.

What it is for

TT2112 is used to submit various driving-licence applications to the Motorisation Office, including applications to obtain a category. The form number alone does not tell you which procedure you are starting: the Motorist Portal provides information versions and forms for different applications. To avoid rejection, first identify the category and type of request, then download the attachment linked to the relevant MIT guidance.

The form collects identifying details, the category, the requested procedure and required declarations. Some fields depend on the procedure: a motorcycle test may require vehicle details, while a duplicate or renewal requires different attachments. Do not reuse a copy filled in by a relative or saved from a previous application without comparing it with the current version.

Fill it in accurately

Enter your details as they appear on your ID and tax code. Check your first name, surname, place and date of birth, address, requested category and contact details. If the form requires the applicant's signature and, for a minor, a parent or guardian's signature, include every required signature. Leave office-only fields blank unless the instructions say otherwise.

Before signing, read the notes and declarations: ticking the wrong box may route your application to a different procedure. If you are unsure how to answer a field, ask the UMC instead of guessing an abbreviation. Do not make unclear handwritten corrections: download a clean copy and fill it in again, or ask whether an amendment is allowed.

Attachments and payments

Submit the form with the documents required for the category: these usually include ID, tax code, an existing licence when required, a medical certificate and PagoPA receipts. Amounts and payment codes can change, and some local procedures may differ, so make payments through the Portal or follow current MIT guidance. A receipt for a different procedure does not make your application valid.

Keep a complete copy of the signed form and attachments, together with the submission reference. If someone submits the application on your behalf, check which ID and authorisation are required. Non-EU/EEA citizens, conversions and renewals may require additional documents: the standard checklist does not replace the list for your specific procedure.

Where to find the correct version

Open the MIT page for the category or procedure and follow its link to TT2112 or the Motorist Portal forms. Check that the file is issued by the Ministry or the Motorist Portal and covers the right procedure. If your UMC requires a particular submission method, follow its local notice. Filling in a form does not mean the office has accepted your application until it registers it.

Frequently asked questions

Can I use a TT2112 form I found online years ago?

It is safer to download the version linked to your current procedure from the MIT or Motorist Portal, because forms and attachments change.

Is the TT2112 enough to submit an application?

No. You also need the attachments and payments specific to your category and procedure.
