Licence application: can a health card prove your tax code?
MIT accepts a copy of the tax-code card or health card showing it; check that the details match the rest of your file.
For a B application, MIT accepts a photocopy of your tax-code document or health card showing that code. Check that your name and code match your identity document and application; the health card does not replace the separate identity document requirement.
Which tax-code document to attach
MIT’s B licence document list asks for a photocopy of the tax code or health card that certifies it. You can therefore use the health card for this purpose if it is legible and the details match. Provide the required copy and keep the original available in case the office wants to inspect it. Do not confuse the tax code with identity: MIT lists these as separate file requirements.
Before filing, compare the code and personal details with the application, identity card and medical receipt. If your name has changed or the code does not match, do not alter the card or form by hand; ask the UMC which official document proves the correction. If you lost the health card, you may use a copy of the tax-code document or ask the Revenue Agency for a replacement. A blurry or cropped image may not be readable at the counter.
Prepare the other documents
The tax-code document is only one part of the application. Also check the TT2112 form, valid identity document, medical certificate and fees for your route. The MIT page may be updated and different stages may have different tariffs; confirm the applicable code and keep the right receipts. If a representative or driving school submits the file, ask whether an extra copy or authorisation is needed.
If the UMC finds an inconsistency between the tax code and identity document, ask which part of the file must be corrected before resubmitting anything. Do not pay a new fee to replace a copy unless the office asks.
